In Ohio you pay a fee every year to renew the plates it's like 20 or 30 more then the regular plates
I realize that it might be different from state to state, but is a vanity plate a one time fee or do I have to pay more each and every year when I renew my registration?
I got the feeling from the website that there is no additional fee each year, but I am not sure if that is right. I don't want any surprises come renewal time!

In California you pay a fee to get it and then you pay a lower additional fee each year to keep it. It wasn't very much the first time when I got mine and it's a small amount extra each year. It didn't cost much each time I've transferred it to new cars either.
You pay more each year at renewal.
I am trying to get rid of mine for that reason, and it has proven to be the most difficult thing in the world! Ugh!
Depends on the state. Some have a one-time fee and some it's annual. The cost also varies. MD is much more $$ than VA, which is why everybody in VA seems to have a vanity plate.
according to your state's (if you live in PA) you pay a $20 fee. It's not clear if it's every year or just once. I would assume it's $20 per year.
The NJ website makes it look like its a one-time fee also, but I'll let you know next year when I get my renewal :)
Edited to add: I just went to the website to check again, and for NJ it specifically says "for a one-time fee of $50", so I really do think that is it for mine until I have to replace them in a few years.
Can you look your question up in the PA Department of Motor Vehicles & Safety website?
In NV we pay each year for it. It doesn't go up, we just pay an extra $10 or something.
Not sure about the vanity plates, but the specialty plates that support wilderness or have the TX flag on them do cost more annually.
I'd assume the same with vanity plates
