I wouldn't use any sort of cream on it either. Sounds like baby acne, but to be sure, call your dr.
I would be more worried if baby seems to be bothered/irritable because of it.

I plan on calling the pediatrician on Monday, but my son who is 4 weeks old, has developed over the last couple of days a rash on his face. I think it's probably baby acne, but I'm not sure. It looks like tiny red bumps mostly on his cheeks and neck. Some of them do seem to have little fluid filled heads, but some are just little red bumps. The part that confuses me is they will fade in and out. So sometimes - especially when he's crying or upset or a bit too warm, they will appear very distinct red/dark pink. Other times they will fade and some will be skin colored - although you can still feel the raised bump. Does this sound typical for baby acne?

It could be keratosis Pilaris (kp) and if so, will likely be outgrown.
Lots of babies have imperfect complexions.
Definitely ask your pediatrician about it.
Personally, I would only use water on a 4 month old's face.

4 weeks is the peak of infant acne.. it is related to your hormones leaving the babies body.
both my kids had it .. I did nothing and it went away. a while ago.. someone asked what to put on it.. and everyone had a different cream or ointment thy used and every baby got better in a few weeks.
makes me think that they would have gotten better without any treatments.
call the dr.. but most likely infant acne.. and it will get better in time.
My first had something like that, it was a little rash, not acne and my doc gave me this awesome cream for it and it cleared righ up. I think it might be a rash bc it changes in appearance at times so hopefully your doc can help.
My dd had the same thing...caused by the shampoo I was using. I put a tiny bit of hydrocortisone cream (1 percent OTC) and it went away. My pediatrician said not to use soap at all, to just bathe in water.
I wouldn't do anything except wash his face with clean water. Don't touch the acne at all or you risk breaking the skin and causing an infection or scarring. It's most likely your hormones leaving his body.
