Hoosier healthwise is a lot less paperwork than Medicaid. I was on it for my first pregnancy, and if I remember right, I needed pay stubs (3 0r 4 months worth) for everyone living in my home. I also needed proof of pregnancy, which has to come from a doctor, the usuals (drivers license, birth cert, and ss card), and bank statement. There was a couple of other things too, but if you call, they can tell you over the phone. The first thing I did was go to the office and fill out paperwork. The office in Fort Wayne is at the corner of Rudisill and S. Clinton St. You fill it out there and hand it in, and then you leave and are sent something through the mail that has an appointment time and says exactly what paperwork you will need to bring. It's at that appt. that you will show all of your papers and they will determine eligibility from there. I would definitely call first though, because that was 5 years ago, and by now, you may be able to access the initial paperwork via the internet to save you a trip down there. WIC and medicaid, like someone else was mentioning are completely different. When you apply for hoosier healthwise, you're only applying for hoosier healthwise. It's different from medicaid applicants. Actually, hoosier healthwise is a lot easier to apply for and be accepted into than anything else because it's purpose is to insure children. Once you have the baby and have your 6 week checkup, you will be dropped and your baby's birth will need to be reported so that the baby can be covered. Don't worry though. The whole process for H.H. is very easy and only has to be renewed once every year.
