I have been there 5 times. My husband was born & raised there thru college. so we now visit every year & bring our daughter. Its a beautiful island but very americanized. every restaurant we have here~ they have. everyone speaks some English. Weather is beautiful always 80's never too hot or too cold. The touristy place to be is Old San Juan, if they don't have a hotel yet, I would recommend the Marriot or the Caribe Hilton. they are close to everything & very walkable or short taxi ride to the sights in downtown with the forts, castles & old brick streets just like in spain. These hotels have casinos & nice private beaches. catering to american & turists. The downtown is rich in history & beautiful buildings & nice shops & restaurants. There is the old castle, fort (El Morro), paseo de princessa~ a path way below the fort along the Ocean~perfect for nice leisurely walks. a park called "Parque de Palomas" or pigeon park great views & nice place to sit & feed the birds. there is also a free trolley that takes you from one turist spot to another in downtown.
if they want more nature, they can set up tours thru the hotel to visit the rainforest, caves of Camuy, snorkeling trips to the smaller private island & things thru the hotel. it maybe more expensive, but worth the hassle of renting a car & trying to navigate around there. If they are very adventurous then I would suggest renting a car & driving to the rainforest, ect ~ people there drive crazy & streets are not lit well & so it makes driving when you don't know where your going stressful & dangerous in my opinion, & you need to be an expert parellel parker! Old San Juan is a nightmare to drive thru with tons of one way streets, dead ends & very narrow roads~ I would not recommend renting a car to drive it, just use the free shuttle & park at a hotel if possible. We get lost or stuck in traffic everytime we try & my hubby is a native!
there are two islands off the coast Vieques & Culebra that are both great for a day of snorkeling, or sun bathing on beautiful prestine beaches, but not much else. there is an inexpensive ferry that goes from a town called Fajardo to each island daily but Fajardo is about 45 minutes from San Juan.
