I'd go with a gift certificate to a spa. There's something for everyone at most places and if she keeps herself looking good she will probably appreciate it.

Gorgeous monogrammed bag for mom for a cause!
we made (indoor style)stepping stones. fill a shallow cake pan with an inch of plaster or wall compund and have the kids put a handprint, some sparkly stones and their name and date in it. Let it dry and pop it out to give.
I am doing a late (she knows it's coming but not what) collage of photos of her grdds, areas near past homes, etc. There are some awesome frames available now in varying sizes and prices. (AC Moore's, Kmart, Kohl's, Walmart, etc)
{she has 2 granddaughters, our 8 y/o and sis-in-law's 9 y/o}
Get her a pretty corsage and take her out to dinner on her special day (with your husband and kids of course because it's your special day too!).

My daughter is giving her Grandmother a plaster stepping stone, like another poster mentioned. She loves to garden, so we get her a perennial or a flowering shrub most years.
I found a cute tropical-striped handbag, and I'm filling it with plastic summer tumblers, citronella candles and funky plates for my mom to enjoy on her new patio.

How about something she won't do for herself, like a gift certificate for a massage or facial, or a pedicure, whatever she likes. Or take her to lunch for a girls' day out. Or some photos of the kids that she can put in her purse and show off to friends.
I always like to get her a gift certificate to her hair salon.
I like sending gifts from Lush -- they do really nice bath/shower products -- they're luxurious enough that people generally won't buy them for themselves, really high quality, made locally to their stores, and consumable so you can get them again and again if the recipient likes them (we've had requests for repeats from everyone we've given them to so far.) www.lush.com
We're getting my MIL a digital picture frame, pre-loaded with pictures of her family. For my mom, who lives out of state, we bought and printed out some gift certificates to restaurants in her area from restaurant.com for a fraction of the price you'd pay directly from the restaurant.
So I have 2 suggestions. I've been taking Mona Vie whic is made from the Acia berry in Brazil. It's jam packet with antioxidents and nutrients and completely about health, which sounds like she maybe.
Or my favorite gift to give is a hand-made project with pictures. Whether it be a decorated photo page to frame, a photo box or small photo booklet.

A certificate for a facial and a manicure. This way if she is uncomfortable with a massage, this will be good because she won't have to get undressed! And don't skimp on the price with WHATEVER you give her. Think of all that she does for you, and especially that she gave you her SON! :)
We like to do crafty things that are inexpensive, personal adn fun. One of the grandparents favorite that we did a few years back was the kids painted small flower pots. Then they pick out a plant or flower to put in it. Grandma likes red so she got a red flower. Nanna loves tulips.
