My husband and I are going out of state for a vacation without our 1 year old. She is staying with my in-laws. My MIL suggested we get a consent to treat form notarized in case there is a medical emergency with the baby. Does anyone know where to get this? The hospital in their area? Our pediatrician's office didn't have one. They suggested just writing up our own. Any other information out there would be helpful!

You can just type up your own & take it to be notarized at the court house in your area or if you no anyone that is a notary they can do it for you...this is something that i do for my son anytime we are out of town & he stays at home. All you need to put is the dates that he will be under your MIL care & state that she is the only person able to have authority. Hope this helps!

Hello! We had a consent to treat letter for our children when they would stay at the babysitter's house or were out our parents house. I just made a letter on our own letterhead and stated that our inlaws were responsible for our children at that time. I stated our children's full names, dobs, and our in laws names, then also included the insurance information on the letter. My husband and I both signed it. We did not have it notarized, though.
