Horrible Diaper Rash - Grand Forks AFB,ND

I need some serious help. My 15 month DS has a really bad diaper rash that has spread to his genitals. He cries during diaper change (obviously) and I don't know what to do to make it better for him. I stopped using wipes and started using wash cloths, I was using butt paste and then switched to desitin creamy because the butt paste was making it worse. He screams when I use the desitin as if it burns. The diaper rash only just began three days ago, the day before a high fever. My DH and I are relating it to teething, they're just popping up left and right. He's never had a diaper rash that didn't go away almost instantly with butt paste. Any advice??

What can I do next?

I had this problem with my daughter since she was on constant antiobiotics for ear infections. This is a recipe from the doctor: 1:1 ratio

Lotrimin (or we used the generic clotrimazole cream)
hydrocortizone cream

Just mix equal amounts of each.

We had tried 3 different prescription diaper ointments and this finally cleared it up.
Good luck

I always use those tubs of Eucerin cream on my kids' butts. It's not so full of chemicals like the other stuff is.
If it is raised red bumps then it's probably a yeast or fungal infection. Take him in and get a prescription. I know others have suggested trying this or that but if it were my butt, I'd want the right stuff now, not 3 days from now :o)
Best wishes,

You should go and see baby's doctor because what started as a diaper rash may have evolved into a really bad yeast infection. Since baby also has a fever, it could also be infected now with bacteria and since that area is always wet, it does not have a chance to heal.

Poor pumpkin, I'm sorry as well if anyone has mention my option but I've been told by a ton of people and use it and its great is corn starch. Get a bottle poke holes big enough and use it like baby powder, just make sure you always have the infected area dried first. My sister-in-law called me stating her son had a sore in a crease of his neck from a issue he had w/ his neck, if it didn't get dried after baths or anything wet it would start to smell and turn red told her keep it dry and put corn starch on it through out the days and it took care of it. Good luck

Like the others, I would agree it is likely a yeast infection easily treated with lotrimin. Don't use wipes for a few days, just a wet wash cloth to wipe so it doesn't burn. My son gets them every now and then and I can usually tell when the diaper cream doesn't seem to help within a day. I try to make sure I give him yogurt at least once a week to help as well. It becomes really common with ear infections too because of the antibiotics.

Lots of cool baths (not too cool, but not too hot, either) and lots of naked time outside! My first hardly ever had diaper rash, and I attribute it to the fact that he was born in Washington, where it's nice enough to be naked outside a lot more than in Minnesota! :)

It sounds like a yeast rash to me. We never had diaper rash issues here because we used cloth diapers, but I DO know that Desitin is really harsh and most kids react to the zinc oxide and it burns, so please dont' use it on an open rash, its very painful!

If it is a yeast rash, start him on acidophilus probiotic. This will fight the bad bacteria and replace it with good. Get some Lotrimin and put it on his rash, it will fight the fungus. Keep his butt clean and dry, give him some naked time in the sun, and let him sit in a sink of lukewarm water after a diaper removal for a few minuts, then use a hairdryer to blow his butt dry... the warm air will help.

Try letting him go without a diaper for a while. Spread an old blanket outside and just let his butt air dry and breath. I also use regular corn starch on my son to powder his butt between each change. He has a super sensitive bottom and I found this to work better than any diaper cream. If it lasts even a day more, I'd take him to the doctor. They have prescription creams that can work miracles and just to rule out the possibility of it being any other kind of infection.

You may want to have him checked to make sure it's not a yeast infection. It can look very much like a diaper rash and affect boys and girls. If it's not that then your dr can prescribe something stronger for the diaper rash. Also just keeping his diaper off as much as possible can help a lot (hard I know for a 1yr old teething). Hope this helps :)

if you want to save a trip to the doctor then get some anti fungal cream at the drugstore and mix it half and half in your hand with diaper rash cream and put it on there. that is what my doctor had me do when my son had a yeast infection. he didn't give me a perscription or anything.

If it's bad enough though, they do have a cream they prescribe but i don't know what it's called. My cousin gets really bad diaper rashes and my aunt has some that her doctor gave her.

Your son may actually have a yeast infection. Yes boys can get them, too. When you change him let him "air out" for about 15-20min before putting another diaper on him.

You may want to double check with your Dr and see if it's a yeast infection. (I know, I thought that sounded crazy with a boy!) I fought what I thought was a "bad diaper rash" with my 1st boy and tried just about every cream and combination of things to clear it up (no wipes, just washcloths, air drying, no diaper, quick dips of his buns in the bath, this cream, that ointment, etc., etc.- until FINALLY a Phys Asst friend said, that's yeast infection! I used a Lotrimin product on it and it went away. Another time I needed a prescription to clear it up. He was just prone to yeast infections down there. Once out of diapers he hasn't had any problems with it. Might be worth checking into....

I see that you have a ton of responses, but i will throw in my two cents as well. What I do with my son is change his diaper more frequent than you normally would and when I do I use 100% pure cornstartch and desitin creamy. His diaper rash goes away usally after treating his bottom 3-4 times. I also let him run around with no diaper for 15-30 mins to let his bottom air out and that helps a ton. Good luck!

Hi K.,
My daughter has had terrible diaper rash occassionally and I have narrowed it down to Pampers diapers. Everytime I buy these she gets a bad rash. Since changing her back to Huggies she has been 100% better. I also use Aquaphore with every diaper change. It really helps to block the wetness and helps heal the rash.
Has your child been on antibiotics? It could be a yeast infection as well. Check with your primary care provider, they can prescribe an antifungal for this.
Good Luck!

I know my pediatrician told me to try a home-made paste of liquid antacid (like Mylanta) and cold cream (you know the jar of cream for you face). So I would use that at times, and then sometimes just plain powder works (even cornstarch if you don't have baby powder, and it's just plain and natural).
Hope something helps!
Oh, also if there are open sores my pediatrician once recommended Neosporin (or an off brand of that kind of thing) for right on the sores.

My apologies if this is a repeat, but if it is not a yeast infection or other fungal rash- try Crisco.

Really.

My nephew had diaper rash like I have NEVER seen on a child. His skin was so raw it was bleeding. Diaper changes were an absolute nightmare. even sitting him in the bath was nearly impossible. She had been using tiger balm, which is supposed to be one of the best creams for that ever, and is what her pediatrician had finally suggested, but even that wasn't h elping. She said within 24 hours of the first Crisco application, the rash was nearly GONE. I would not have believed it if I hadn't seen the rash before and then seen it again a few days later. it was amazing. Sorry to gush, but I usually don't believe those old wives tales- and neither does my sister- but she was desperate, so she tried it.

As always, of course, consult a pediatrician or at least call the office and ask to speak with a nurse.

Best luck...

Hi K.:
Have you made a list of what he eats? My grandson cannot eat yogurt. Everytime he eats yogurt he gets a very sore bottom. Even the little yogurt bites cause diaper rash. My daughter took him off all yorgurt products and his bottom cleared up and no more diaper rash.
It may not be yogurt that causes the diaper rash on your son but it could be food related. Make a list of what he eats daily and see how that effects his diaper rash, then remove any foods you suspect are causing the rash.

It is possible that it is a bacterial infection rather than a normal diaper rash. Try putting some Lotrimin on it (yes, the stuff for feet) and letting your child "air out." If it doesn't clear up in a week see your child's doctor.

YeastInfection, Teething, has to be either or. I've found that changing the soiled diaper sooner rather then later is effective along with prescribed medication, when cleared up I've used vaseline on my 5children the oldest is 22 yrs old and the youngest will be 8 months tomorrow (07.14.09), they do make baby wipes perfume free, and use vaseline nothing more but straight vaseline and its cheaper then most OTC ointments. Good luck and hope your little one feels better soon.

K., I actually tried everything for my daughters diaper rash, and what I found to work was when she went down for a nap I put a water proof pad under her, along with a diaper underneath her booty (not on) and let the air do its job. I also at night I did the same thing until I went to bed then I put the cream on her and secured the diaper very loosely. It worked wonders. I don't know if you feed him juice, but make sure you are watering it down 60-40 with water otherwise the citrus will really irritate the skin. Good luck.

I would take him to the doctor! We had something similar (got bad really quickly like yours) and the doc prescribed some antibacterial ointment (2% muripocin) that made it go away in just a couple of days. I think the rash had some sort of secondary bacterial infection - this antibiotic is effective against staph, which definitely won't go away with regular diaper cream. A great relief for both us and the baby.

I think you probably need to consult your ped. soon. I'm surprised the butt paste isn't working, so I'm wondering if he's having some type of allergic reaction to something in it....? Either that or maybe it is some type of virus since he also had a fever. i really don't think it's teething.

I'd try bathing him in a lukewarm bath with oatmeal, (aveeno), bath and then let him run around naked for awhile to give him a break from all the moisture created from the diaper. When you do put the diaper on, try just baby powder with corn starch. It may not help the diaper rash, but will probably help the pain a bit and keep him dryer. Washcloths are a good idea instead of the wipes like you are doing, but dry his butt after-that moisture is going to add to the rash getting worse.

Good luck!

Thanks to three kids of my own and day care, I have a drawer full of diaper rash creams! I've found that with each kid and each rash, my experience is different. For example, all my kids have reacted to the zinc oxide in Desitin and other regular diaper rash creams. My oldest used to scream and scream until I figured out it really was stinging his bottom. So I sometimes use it with other kids (it apparently doesn't sting everyone) but never with my own kids now. So I'm not surprised if it's stinging your son; it certainly did to my kids.

I've had good luck with Butt Paste with mild rashes, ones that look like a mild sunburn. But if they get worse than that, I go to the A&D. It looks and feels like Vaseline, so it provides good coverage (just melts into the diaper too, so you don't have to smear it around and further irritate the bottom) and doesn't hurt. Plus it has vitamins A and D to speed healing. It's what our pediatrician always recommends.

If it's getting worse rather than better, I would suggest a trip to the dr. I've seen the special mixed diaper cream ointments and they've worked wonders. One day care kid I cared for had a terrible, terrible rash (the worst I've ever seen) and got a mix from the dr that was, I believe, Desitin, Lotrimin, and antacid together. It took care of it really quickly. Your doctor may be able to assess whether there is yeast and whether you need something else too.

You can do the basics..keep it clean and dry and let him go without a diaper to "air it out" as much as possible. Make sure he is not eating any foods that could make it worse. Put 2 tbsp baking soda in a warm shallow bath for him several times/day. I would not even wipe him with a wash cloth because that is actually going to rub more skin off and make it more raw...I would simply take him to the bath and rinse him off each time he poops and let him dry off really well. You don't need to wipe when he pees. When my son has had really bad rashes I have also fed him more binding foods like cheese so that the poop is less likely to cause an issue- but be careful if you try this because you don't want to make him constipated either!

My son had a yeast diaper rash about a month ago and was the same way...does your son's rash look a little bumpy and splotchy, really red and painful and has clearly defined borders? if so that could be what is going on. Yeast rashes don't go away they just keep getting worse and worse until you treat them correctly. His doctor had me get an anti-fungal jock itch cream [Lotrimin] for it and it went away in about a week.

Either way I think it is worth it to bring him in to see a doctor. If it's not a yeast rash, they can at least prescribe you a special kind of diaper rash cream that is mixed in the pharmacy. I am in Minnesota and have gotten a diaper cream from Children's Hospital that was THE BEST EVER. It completely got rid of all of my son's rashes overnight. Unfortunately I ran out of that one but next time I take him to Children's I am demanding another tube of that stuff! I have also gotten one that was mixed in a Walgreen's pharmacy. Also far superior to the OTC ones. I don't know what they put in it, but they are so much better. Also petroleum jelly doesn't sting when you put it on like the other creams do. You might want to give that a try. But again, if it is a yeast rash, the OTC creams aren't going to help and medicated cream will be needed.

yes, bad diaper rash probably due to teething (something in the urine is more acidic). My son also had this - ugh! One thing we tried (was suggested to us) and it worked - we used a hairdryer after diaper changes to make sure all was super dry before applying any annointment. Make sure the dryer is on 'cool' setting. I think this also feels good to the little one - having air blown like being au natural (which I think kids like in general).

If the rash is not reponding to the Butt Paste, you may want to try an antifungal cream as he may have a yeast infection. I had the same problem with my daughter, it was a horrible feeling to not be able to relieve the pain when changing her! So I bought some over the counter cream - lotrimin, or any yeast infection TOPICAL creams - ask the pharmacist at the store and they can help. She had immediate relief as it didn't sting or burn. Thank goodness! Also, I use plain old petroleum jelly/vaseline for her diaper rashes and it works great! Cheaper and doesn't have all that other stuff in it. I hope this helps, take care!

The one suggestion I have is not so much for the rash, but for the applying of the cream. I spread the cream on the diaper instead of directly on the skin. Once you put the diaper on it hits the skin anyway, but it isn't as much of a struggle (or mess) for you or as "traumatic" for your child.

Hi K.,
I have had success with 2 things.
1) Apply Plain yogurt on his butt. What I did, was take the container out of the fridge, Put a bit in a plastic (used yogurt container) to warm up almost to room temp.(usually about 5-10 minutes out of the fridge. I put a towel or 1 down and just let him sit in it. If he eats it, that's ok too. It's like magic! Also, after that a gentle wipe down with a really soft cloth, with just plain water to get it off. You can almost see the redness disappear. Then, encourage him to be naked in the sun for a few minutes. I would put a blanket on the floor by a nice sunny window, he would lay down, so his butt was in the sunshine and we would play for a while. If you are nursing, be aware of the acidy foods you are eating. Major diaper rash is a direct reaction of a PH imbalance(more to the acid side) that's why the yogurt works.

Also for teething, Hyland's Teething Tablets are a homeopathic teething item. Read the instructions, I used to give my son 2-4 a few times a day and the crankyness disappeared within minutes.

Have fun!

I did daycare for 5 years and plus with our 2 kids I've tried pretty much every cream out there..... A&D worked for awhile, Butt Paste worked for awhile, Arbonne NEVER worked for us, Gerber brand NEVER worked for us,Desitin NEVER worked for us, etc, etc. NONE are as good as:

HYLAND'S HOMEOPATHIC DIAPER OINTMENT!!!!!

Our son was so sore he couldn't even walk without crying and Hyland's makes a HUGE difference in just ONE diaper change!!! I don't waste money on ANY of the other kinds now!

I buy it online through azurestandard.com but I'm sure it would be in the natural/organic section of a grocery store as well! Good luck.

Sound like your dealing with the same issues I did with my son. He's now 18 months old and we finally have the diaper rash under control. I'll tell you what I did and maybe some of it will help your little guy. First, I started making my own home-made babywipes. It has not only saved us money, but they are so convienent. I will include the recipe below. Second, my doctor recommended mixing half and half anti-fungal and hydrocortizone creams and applying it for seven days at every diaper change (this should be repeated whenever the rash reappears). Be sure to give his bottom lots of "air" time at diaper changings. This is vital to the healing process. It can get messy and you might have to follow him around with a rag and carpet spray :( Finally, I found the most amazing butt cream called Weleda. The web link is:
http://www.luckyvitamin.com/brandListing?keyword=weleda&a...

This website has the best prices I can find. As you look to the right of the page, the orange tube is a wonderful cream to use at each changing and the white tube I use at night before bed. I have also found that unlike my daughter, my son needs lots of baby powder on his diaper at every changing. This has done wonders for keeping the wetness away from his skin. I know all of this seems like a lot of work, but its well worth the effort when you get your happy little guy back.

How to Make Homemade Baby Wipes

Here's How:
1.Cut the roll of paper towels in half with a knife and remove the inner cardboard core (use a butter knife to loosen).
2.Mix liquid ingredients in container.
3.Place the paper towels in the container with the cut side facing downward.
4.Secure lid tightly and allow to sit until all liquid is absorbed into the paper towels.
5.Turn the container upside-down and allow to sit for 10 minutes.
6.Open container and 'start' the roll by pulling a towel from the inside of the roll.
7.Wipes are now ready to use.
Tips:
1.These also make a great gift for new parents. You can decorate or personalize the container and, of course, be sure to include a copy of the recipe.
2.Save the other half of the roll for use next time you make wipes.
3.Make sure the container is closed tightly after each use to prevent drying out.
4.When your baby is older and his skin is less sensitive, you can add about a teaspoon of rubbing alcohol to the mix.
What You Need:
• Round or square plastic container
• Thick, strong roll of white paper towels
• Sharp knife to cut paper towel roll in half
• 2 cups water that has been boiled and cooled to room temperature
• 2 Tablespoons baby shampoo or baby wash
• 2 Tablespoons baby oil

Good luck, I hope this helps. I totally understand your frustration.

My son gets diaper rash pretty bad when he eats things which too much acidity in them. (citrus fruits, some raw veggies, etc.) He is also allergic to amoxicillin. At any rate, here are some remedies that we do when he gets this way:

We feed him yogurt almost daily because the live cultures in it helps to regulate the yeast in his body.

Give your son a lukewarm bath with baking soda. Just let him sit for a few minutes in the shallow water.

Try cornstarch instead of desitin or other diaper creams. It will dries out the area and shouldn't burn.

Try to leave the diaper off for short periods of time to let the air get to the area.

If the rash is really bad, it may have turned into a yeast infection (boys can get them too) so you can use lotrimin cream (generic name is chlortrimazole) it is an anti-fungal used for things like at athlete's foot or gyne-lotrimine is used for yeast infections. If I remember right you can only use a little bit of this once a day, so you might want to call your dr. to clarify.

Hope this helps and your little guy gets to feeling better! It is agony knowing they are in pain!

Hi Karen....my oldest son had what I thought was diaper rash that kept getting worse and worse. I finally took him to the doctor and he actually had a yeast infection. It was treated with some special anti-fungal cream and within a day or two, it was completely gone. I was amazed! Just something else that it may be if the diaper rash cream wasn't working. Good luck!! I have also has success with bacitracin on really bad diaper rash, if you haven't tried that.

I have a couple of thoughts. The first is that it may be a "yeast infection" which generally looks like red raised bumps - almost like little pimples. If that's what it is, the dr. can give you some nystatin cream which will take care of it right away. Another possibility is that your child is highly sensitive to certain foods (my kids always get a REALLY bad diaper rash when they drink OJ or eat too much fruit - seriously bleeding type rash. Ish!) If that's the case, prevention is the key - cutting out foods helps but honestly CRISCO is the best butt cream I've ever used! We keep tubs of it in strategic locations through the house (much like you would Desitin, i suppose...) and use it at EVERY diaper change - it keeps the acid off in the first place. The final tip I have is for when it gets really bad: our dr. gave us a small tub of silver nitrate (the cream they use for burns) and we put just a dab of that stuff on the affected skin area. That usually clears up any serious rash in a couple of hours. It's amazing stuff!! Good luck!

OK, this may sound strange but try cornstarch. My Mom used it on me and my sister and when my nephew had very severe diaper rash she tried it and it helped.

But, you may want to call your pediatrician, anytime a rash is followed by a high fever it can be a symptom of something else.

I hope he feels better soon!!

Hey K.,
My now 4 year old had LOTS of diaper rash when he was younger - it seemed like every wet diaper he had brought it on no matter how quickly we changed him. It was pretty severe in that it looked open and almost bloody at times - very painful I'm sure.

The only ointment that ever worked on him was Aquafor Healing Ointment by Eucerin. It comes in tubes or 14 oz tubs (I get the tubs because at $14.00 a shot you want to be able to use every bit). I put a generous amount on him and it healed what he had and since it has a Vasoline like consistency it made a barrier between his skin and any additional moisture which made it clear up faster. I must have tried every tube/paste out there but none of them worked like this. Now that we have our 2nd, I wouldn't think of using anything else.

On other thing that we used to do with our 1st was to let him "air dry" as much as possible. I little difficult with a boy (you have to stay on your toes), but really helps the healing process too. As much air time as you can give them will really help.

Good luck - hope it clears up and you have a happy boy again soon!

Bring him to the Dr!!! We have a perscription for Butt Paste too.

When my son has any type of diaper rash I gently wash his butt in the sink or bathtub to avoid rubbing it with a wipe or anything that would rub against the raw skin. It is so much easier to just walk into the bathroom and rinse his butt to change a dirty diaper than to hurt your baby and hear the screaming.

When my son had a bad bout of diaper rash (he had rotavirus), I used cloth diapers which do help, and did everything possible to keep his skin dry even airing him out. Throw on some vaseline during diaper changes to prevent diaper rash.
