Something my mom does that my almost 6 yo son LOVES, is a treasure hunt. She makes up clues that lead to the next and the next place and at the end is a surprise/treat. He really loves doing that!
All depends on what Grandmom is capable and willing to do...
a board game(I usually send some from home, candyland, ants in the pants, etc), bake cookies (they are usually just happy using the cookie cutters and sprinkles), bowling (bumper os course), baloon volley ball (1 balloon and a little space = 1/2 hour of fun), local playground (weather permitting), crafts (Michaels has a nice selections in their kids section (just make sure it is age appropriate), etc. Enjoy your visit!!
Last year my Mom built a gingerbread house with our boys (4 and 6 at the time) and they had a great time. The boys got to "sample" the frosting and gum drops while they worked and they loved it. You can find kits that include everything. Enjoy!
When my kids go to visit my parents they love to do crafts, puzzles and blocks. For crafts, my mom keeps Playdoh, crayons/markers and coloring books and foam stickers on hand. Sometimes we take along our Colorwonders books and markers (no mess). She has a few puzzles that are 24-50 pieces. We take our own blocks - Mega Blocks - and they see how tall they can build towers before they fall over. Hope that gives you some ideas.
My girls also enjoyed crafts. If grammy isn't up for that, maybe a movie afternoon. Go to the video store, rent "Cars" or something else, pop some popcorn and then spread a blanket on the floor and watch. They always enjoyed simple things.
