I agree with starting a probiotic, there are chewables and there are also powder forms that can be put in just about anything, liquid or food. Just make sure they are kid formulas as their "good GI bacteria" is in different amounts than ours as an adult.
And in addition I would recommend a soft tissue release of a ligament in the pelvis that a specialist trained chiropractor, like me, can perform. It is a hold and release, no big moves, just gentle pressure/stretch and it has made alot of children have better, easier bowel movements. It is great to have the child checked out for medical issues as you have done and then follow up with a few treatments of this type. I have worked with many children and families in your same boat and it gets to be hard for the child as they do tend to "hold" back from going since they relate it with a strain and pain.
