Skip the drugs. I think you're thinking a bit drastically. There is always the possibility that you have a minor health problem that is inhibiting the weight loss. Ask the doc. Throw away your scale and aim for healthy. The weight will come off. You want it slow and steady.
For exercise, go for a walk after dinner or whenever you can squeeze it in. Take two walks a day if you can manage it. Take the twins in a stroller with you. Make it a fun family time and it won't be so tedious. Walk at least a mile - mile and a half. Park at the back of parking lots and walk, everywhere. Skim the gym, you schedule has gotta be hectic enough.
Also if you cut calories too drastically your body will think it is starving and slow your metabolism waaaaaay down. Ask your doctor to refer you to a nutritionist and ask them to help you figure out how many calories a day your should aim for. They will probably recommend 1200-1500 a day.
Try adding four servings of skim milk a day. It is supposed to help you with weight loss and you need the calcium anyway.
Eat more often. Six meals instead of three. Eat the same amount just split it up.
My doctor had me do all of this while pregnant because of my diabetes. I started 50 lbs overweight so they were concerned. I walked out of the hospital, ten pounds lighter than before I got pregnant.
Try getting a diabetic cookbook from the library. You don't have to be diabetic. It will give you ideas/receipes for healthy food that tastes great and you won't find yourself eating the same boring things over and over.
Throw away any spices that are a year or two old. Buy new ones at the discount store. You'll be amazed how much better your food with taste with new herbs.
