3 Month Old UTI

My daughter Haven is 3 months old. I took her to her pediatrician a couple weeks ago because she was running a low grade temp. They did a urine dip in the office and it was negative so they sent it off for culture and it came back a trace of bacteria. So they put her on amoxill for 10 days and now they want to send her for all these expensive tests. Kidney ultrasound and an xray study of her bladder. I don't think these tests are necessary and I don't think she needs the radiation exposure from the x-rays. And I don't think our medicaid will approve it! I want to do what's best for my baby, but she is totally fine now with no further problems. Any advice or opinions?

You have to be careful about not doing what the pediatrcian request. It could be misunderstood. I would tell the Physician exactely how you feel and explain why. Then ask him why he feels these test are needed. Be honest with the Phisician and talk to him, that's what he is there for and if you do not feel comfortable with he or she maybe you need to find someone else. What ever my son's pediatrcian says I trust 100%.. Better safe than sorry. Best of luck. Hope eveything is fine.

Hi K. - Your pediatricians are probably worried about kidney reflux, meaning that there is a possibly a little bit of "back up" into your daughter's kidneys which could be due to a variety of different things, including structural anomalies. This can be a cause of kidney/urinary tract infection, which is unusual in a 3 month old. They wouldn't be doing their jobs if they didn't recommend this and I doubt they are just "running up the tab" because the tests would be done by someone other than them. I think you should discuss with them what they think could be the cause of the infection and why they think the tests are necessary, as opposed to any other testing which could be less expensive and/or less invasive. Finally, I think you should tell them about your finances - many times doctors will work with you, especially if you are up front with them and especially when it involves the health of an infant. I would get the tests done. My daughter was a little older, but had a UTI as well. I was apprehensive about the tests as well, but more times than not, there is a problem. My daughter has kidney reflux stage 2. She did have some minor damage to her kidneys because of the UTI. If your daughter has more UTI's, you are risking her kidneys.

Hi-I am not an expert but I do work in a Pediatrics Unit at a local hospital. We have a wonderful Pediatric urologist who preforms surgeries at our hospital. A 3 mo. old w/UTI is very unusual. Do have the test done. Get referred to a ped. urologist. Prob. testing for urinary reflux which can correct itself but many require surgery. To put a number to it we always have kids in for these surgeries on Mondays and can always expect between 2 and 5 kids every Monday. Please have the test done, the earlier the better in treating this and other kidney problems.
Also, as another mom said, the refusal of test for a 3mo. old or any child can look bad (i.e. some peds. would call social services).

They are probably sending her for the tests to see if she has kidney reflux. My oldest had it when she was a year old, and was put on an antibiotic for a year. the reflux corrected itself and we had no further problems. If your Pediatrician refers you then medicaid should pay. You can always call the number on the back of your card to check. Good Luck!!

do that with my 2 year old but the doc thought about it for a min and said never mind if she gets another uti soon (within the next year) then we will go that route. tell them you want to wait

A UTI in a 3 month old is unusual. I would talk with the doctors about why they want the testing and then see if they would start with the ultrasound first and then if needed go for the x-ray. Either way, I don't think I'd ignore the recommendations of the physician as they don't send babies for these kinds of tests just for the heck of it. The need for the testing has to outweigh the risk of radiation exposure in a 3 month old for them to even suggest having it done.

You know your baby. Be careful to watch for any new symptoms. If the fever is gone and she is back to herself again, eating and sleeping, playful and happy then give it some time. It is always best to find a pediatrician that you "YOU TRUST" and have confidence in. Never hurts to get a second opinion. I believe medicaid pays for "Well Checks". Take her in for a well check, just to make sure.

K., I wouldn't do the x-ray unless it reoccurs. A trace of bacteria in the urine is nothing to be alarmed about. Having said that, if it occurs again, I would consider having the ultrasound and x-rays. There could be some kind of obstruction or reflux that sends bacteria from one part of urinary tract to another.
