I would leave them in place and let them tighten back up. If and when you see a white bubble on the gum directly over the tooth, it's time to try to do pulpotomy to save the tooth in question. Lots of times they put them on antibiotics first to get the infection down and if all else fails, they will extract but DO NOT let him go without replacement flipper (fake teeth). He needs those front teeth for speech and oral motor development of the jaw and face, and self image. I know too many local pediatric dentists that don't want to spend the time to make a bridge for a kid but they will not ger permamant teeth to replace those till around age 7. Bad speech habits will form and the way they say words, is the way they hear words, is the way their reading will be influenced. Get several opinions. But definitely replace the teeth if they need to be extracted.
