Oh girl, I really do feel (well, not feel), but understand your pain!! My baby girl, whp is now 9, had the SAME issues ever since we got custody of her at 2 yrs old. I'm sure it was going on before that too. (I just didn't have her, so I wasn't there) OMG, it is so sad! I tried EVERYTHING, and it seemed like nothing would help. It got to the point where my child was afraid to have a BM, and really, who could blame her?? The problem is, when she held it, it was even worse and more painful when she finally did go! I eventually found that the best course is to make sure that she drinks limited milk (or in your case formula if she is still on tha), and more water and juices. I ALWAYS give her either orange juice (it used to be 1/2 and 1/2 watered down when she was younger so it wasn't too harsh on her tummy when she was younger) in the morning with her breakfast, and then throughout the day she drank several cups of apple juice. She loves to drink water. I don't put anything in it for flavoring. I think that I once added some corn syrup that someone suggested to help with the constipation, but she didn't like it. (she isn't a "sweet" type of kid, and prefers if things are naturally sweet, like juice. If I am really worried because she hasn't had a BM all day and into the next day, (which is the longest I allow it to go now) I give her some Juicy Juice, it's a particular one. They make a citrus blend, and let me tell you, it is like a laxative for her! I keep some on hand just for this reason. I don't give it to her at ANY other time, because it really, REALLY does make her have very loose stools, but in cases where I know that she hasn't gone, well, it saves her some pain and anguish everytime. That is the only thing that has worked for her. I have used the kids suppositories, and even the prescription meds, and they all worked for a day or 2, and then stopped. If she drinks her OJ and the water and apple juice like we want her to, she does ok most of the time. It really is about having enough fluids in their systems. That seems strange since we are talking about BM, but it is the answer. I really do hope that you can find an answer for your little one! This is something that I wouldn't wish on ANYONE!! Oh, and one other thing. I know it sounds very nasty, but when you have to "help" her out when she is having a BM, dont be afraid to use a little something to lube her up there. It wont hurt her as much if she has some vasaline or some lubricant near the opening. (Not way in it, but near the end) Also, I would not reccomend the thermomiter again either. If you need to use anything go with the suppository. It's made for this purpose, is shorter and way more blunt ended. That way you don't need to be concerned about any damage internally.
